Plague Fanatics are part of the Devouring Plague faction of the Withered Aegis and the minions of General Reklar Plaguebearer. When you killed, they will drop Ethereal Shards, which are needed to cast the spell Primal Boon to be able to damage Reklar.
Plague Fanatics are a type of Ghost and are found in Reklar's Fortress in the Eastern Deadlands.
